Overview

•Berberine is a potent inhibitor of CYP3A4, CYP2D6, and CYP2C9 enzymes, significantly raising serum levels of narrow-therapeutic-index drugs such as cyclosporine, tacrolimus, and statins.
•Concurrent use with prescription hypoglycaemic agents (e.g., metformin, sulfonylureas, insulin) increases the risk of symptomatic hypoglycaemia and requires medical supervision.
•Strictly avoid during pregnancy, as berberine crosses the placenta and can displace bilirubin from albumin, creating a risk of kernicterus in the foetus, and avoid while breastfeeding.
•Gastrointestinal adverse effects including diarrhoea, constipation, abdominal cramping, and flatulence are common, particularly when initiating therapy or taking on an empty stomach.
